New Patients
Complete Your Intake Form OnlineWe ask all new patients to complete our intake form before their appointment. This helps us prepare for your visit and ensures Dr. Greenberg has everything he needs to provide you with the most thorough, personalized care from the moment you walk in. The form covers your personal information, medical and eye history, insurance details, and reason for your visit. It takes about 5–10 minutes. Prefer paper? Your Questions, AnsweredA comprehensive eye exam includes a thorough medical history review — including medication usage — and an evaluation of previous eyewear and vision concerns. You'll undergo tests to measure your prescription (refraction), assess your eye health, and screen for conditions like cataracts, glaucoma, and systemic diseases such as diabetes that can affect the eye.
Bring your vision and medical insurance cards, a list of all medications (including over-the-counter and supplements), and your current glasses or contact lenses — even if they're outdated or not working well. We can gather valuable information from your existing eyewear.
Plan for up to an hour for a comprehensive exam and eyewear consultation. If you need additional services — such as a contact lens fitting or diagnostic imaging — it may take longer. We encourage you to allow enough time; some additional services can also be scheduled for a future visit if needed.
Please call us so we can adjust our schedule. We'll do everything we can to accommodate you upon arrival. Keep in mind that other patients may have arrived in the meantime, and if your late arrival falls outside our clinical service window, we may need to reschedule.

Yes — we provide comprehensive pediatric eye exams for children of all ages. Many vision problems in children go undetected or are mistaken for learning difficulties. Early detection through a proper exam makes a significant difference in a child's development and school performance.
Absolutely. Outside prescriptions are welcome for eyewear and contact lens fittings. We're happy to help you find the perfect frames or lenses, even if your exam was performed elsewhere.
